Understanding Therapy Fees & Insurance Coverage

I accept credit, debit, and HSA/FSA cards. Payments are automatically charged the day of your appointment through the patient portal.

My commitment is to ensure that everyone has access to quality care regardless of income. I’m proud to offer a Sliding Fee Discount Program designed to help individuals and families with annual incomes at or below 200% of the current Federal Poverty Guidelines (FPG).

Key Elements of the Program:

• Eligibility:
  The program is available to all individuals and families with incomes at or below 200% of the current FPG. Income and family size is determined in accordance with federal standards and review patient eligibility periodically to ensure continued access.

• Full Discount for Lowest-Income Patients:
  Individuals and families with annual incomes at or below 100% of the FPG are eligible for a full discount on our services. In some cases, a nominal charge may be applied. Please note that the Department of Health (DOH) reserves the right to determine the reasonableness of any such nominal charge prior to approval.

• Adjusted Fees for Those Above 100% up to 200% FPG:
  For patients whose incomes exceed 100% but do not surpass 200% of the FPG, fees will be adjusted based on both income and family size to ensure fairness and accessibility.

Yes, I accept a variety of insurance plans. I work with major insurance providers, including HMO, PPO, and EAP plans. Please refer to the list of accepted plans below or contact my office to find out if your plan is accepted. While the list is not exhaustive, it’s a general overview of acceptable plans, but is subject to change. Please reach out to confirm your plan is accepted:

It’s best to contact your insurance company directly to confirm your benefits and understand any out-of-pocket costs. I can also assist by providing my provider information if needed.

If in network, I will submit the claims on your behalf. There are times, however, when certain plans require submission from the member to be reimbursed. Your insurance provider can explain what your plan allows for claims processing.

Your insurance plan may require a co-pay that is collected at the time of service. Please check with your provider to understand your specific requirements before your appointment. The co-pay is automatically charged to your credit, debit, or HSA/FSA card on file on the day of your appointment.

If your insurance benefits are limited, I offer flexible payment options, including a sliding fee scale, to ensure therapy remains accessible.

Yes, I work with both in-network and some out-of-network plans. If you choose to use an out-of-network provider, please consult your insurance policy regarding reimbursement rates and procedures.
